2011 Unlimited Season

PLEASE REGISTER ON-LINE FIRST!!

   To Register:  Please follow the below steps.

  1.  Scroll down to "click here to register".

  2.  Create a family account, enter all of your information and create your own login

       and password.  Record it for future use.

  3.  Use your new login and password to Register all of your children.  Please follow

       onscreen instructions as they will guide you through the registration process.

  4.  Go all the way through the payment screen where you can choose to pay by

       credit card or by check.  If paying by check you must bring your check to one of

       Paperwork turn in days. 

  5.  The payment receipt is the last screen and it will be emailed to you as well.

  6.  On the payment receipt screen there are 2 adobe (PDF) documents: 2011 Player

       Contract and Medical Waiver - click on the links to open the documents and

       then print both of them.

  7.  The Player Contract (2 pages) must be completely filled out and signed by both the

       Parent and the Participant (Note: some information is filled out already by your entries

       but other items need to be filled out by hand).  Bring this to a Paperwork turn in day!

  8.  The Medical Waiver must be signed and stamped by your physician.  Please keep

       both pages of the medical form until it is signed by your physician and then turn in

       both pages together. 

   9.  All new participants need to bring a copy of their birth certificate for our files to keep.

 10.  Bring the completed 2011 Payer Contract, Payment by Check, and Birth

       Certificate (if a new participant) to one of the following Registration Paperwork
