2012 Football and Cheer Registration May 1-June 9
Please click on the link below to register for the 2012 season.
Registration is a 2 step process.
1. Register Online. Fill out all fields that are asked. Before logging off, please print out registration forms and fill out any missing information that has not been filled out. If you have difficulty registering online, please feel free to attend one of our Paperwork Turn In Dates and we will be happy to help you through the online registration process.
2. You will need to bring your forms and outstanding payment (if you do not pay online) to an in person registration paperwork turn in. Registration will not be complete until all of the following forms are turned in:
Birth Certificate (only participants new to the program)
Registration Form
Image Release Form (Minor)
Emergency Medical Consent Form
*Medical Clearance Form
*2011-2012 Full Year Report Card
*These two paperwork items can be turned in at Uniform Pickup. All other forms to be turned in at the registration paperwork turn in.

The Community Center is located at 1515 W. Touhy Ave., Park Ridge, IL 60068.
Registration Fees
Fees: $425 per participant
Family Discount: $375 for second child, $325 for all other children in program.
